1. Job Summary: The Health Care Liaison provides support to the facility nurse(s) who provide supervision of health care and support to people with developmental disabilities who reside in Laura Baker residential facilities. She/he represents Laura Baker Services Association directly to the customers, using Laura Baker Services Association standards of conduct and guiding philosophies to promote the well-being of the people the Association supports. The Health Care Liaison will coordinate the scheduling of communication of, and supervision needed during medical appointments.
2. Conditions of Employment:
- Must pass a Minnesota Department of Human Services background check and must be able to maintain that eligibility. Must be fully vaccinated for covid-19 or receive exemption. She/he must be able to perform the environmental, physical, mental, and sensory demands of the job. Upon hire, she/he must be able to prove valid work status and complete manteaux screening.
- Must maintain a valid Minnesota driver's license.
3. Reporting Responsibilities: The Health Care Liaison reports to the Registered Nurse She/he is a member of the health team and Oak Street Services Management Team and is responsible to regularly attend the scheduled meetings of those teams as directed by the Director of Nursing. She/he provides supervision of health care appointments and reports any concerns.
4. Internal Contacts: The Health Care Liaison will primarily interact with Laura Baker Services staff members and the people she/he directly supports. Primary contacts will be with the facility nurse(s), the unit staff, clients, the health team, the Behavior Analysts, Household Directors and the kitchen staff.
5. External Contacts: The Health Care Liaison will interact with any and all of the constituencies of Laura Baker Services Association and the community at large. Primary contacts will be with Day programs, and other medical professionals, as she/he assists clients to schedule appointments.
6. Responsibilities and Duties: The Health Care Liaison is responsible to ensure that the following responsibilities are completed promptly and to the best of his/her abilities:
- Communicating with various aspects of the Laura Baker Association Services and outside individuals, including day programs, Night Residential Counselors, support personnel, medical staff, parents, social workers, administrators, and co-workers, medical professionals, pharmacists and dietitians under direction of facility nurse(s) or consulting RN.
- Act as a liaison between medical professionals as an advocate of the client. Assure needed medical appointments are scheduled. Attend appointments as needed and assure necessary follow-up is done. Communicate appointment times and date to all appropriate staff.
- Participating in interdisciplinary team meetings, health staff meetings, training sessions, OSSMT and workshops as directed by the Director of Nursing.
- Driving clients to and from appointments
- Adhering to the Laura Baker Services Association abuse and neglect policy.
- Adhering to the policies and procedures outlined in the Laura Baker Services Association Policy Manual.
- Performing other appropriate duties as assigned by the administrative staff.
7. Decision-Making Responsibilities: The Health Care Liaison has the authority to make decisions as outlined in the "Levels of Decision-Making: Health Care Liaison".
- Representing the Association in a positive and professional manner.
- Client and staff concerns to facility nurse(s) and the consulting RN.
8. Knowledge and Education: The Health Care must maintain current CPR certification. She/he must promote Self-Determination by respecting the life choices and dreams of the people she/he supports. Experience and/or education in the field is preferred.
9. Skills and Abilities: The Health Care Liaison will have the skill and ability to work independently as a member of the health team without constant supervision; communicate effectively with clients, co-workers, other medical professionals and the IDT members. She/he will act as a professional role model. She/he must possess organizational and time-management skills and maintain confidentiality.
The Health Care Liaison will be able to meet the qualifications noted on the Environmental, Physical and Mental check lists.
10. Accountability: The Health Care Liaison is directly accountable and responsible to the Registered Nurse
